Who pays ATM usage fees?

Why should I care what the ATM usage fees are? I haven't paid one in the United States since -- well, probably since the last time I moved from one state to another, which was twelve years ago. The only time I pay an "foreign ATM fee", as the bank statement puts it, is when I'm actually in a foreign country and therefore can't do the 'cash back' trick, and that's extremely rare.

Seriously: does any basically responsible person actually pay these fees, rather than just getting some cash when you're buying groceries, or going to your own bank's ATM machine (which is always free)? So why the heck do we care whether the fee has gone up?

It's like worrying about what your long-dead great-great-grandmother might have thought about your hairstyle: even if you know the answer, who cares?!
