Reclining Car Seat Necessity

Because of a back injury which was damaging my sciatic nerve, I could only sit long enough to lie down. Because of this, whenever I had to go somewhere (doctor's appointment, physical therapy, etc.), I was forced to recline fully in the front seat while someone drove me. I often wondered how safe it was to ride in such a manner. What do the car manufacturers have to say about such a condition? Are people with physical problems such as mine supposed to risk more serious injuries because they cannot come up with a better seatbelt configuration? Do they think such conditions occur too seldom and/or are short lived and not worthy of consideration? Does 13 straight months sound short lived? It only takes one ride lying prone to have an accident.
