There's a sinister ending to these experiments in financial incentives replacing social coercion, like the one where parents were fined for being late in dropping off their kids: it turns out this change is irreversible. When the fines were removed, because they actually made the problem worse, the parents didn't go back to their previous ways - they remained as likely to be tardy as when they were incentivized to be on time.

Once some action is taken out of the social sphere into the economic sphere, it will take a generation to reverse the consequence. That means that removing incentives from medicine will not all of a sudden make doctors only care about their patients - just as it won't restore patient's respect for their doctors.

Maybe it is the only solution, but if it is, let us not have any illusions - it will take decades before it starts to work.
