That's the problem in a nutshell. From my time working for one, I could plainly see that insurance companies are expensive generators of red tape, and unless you are a VIP customer, they ohave to deny you coverage as much possible to make their profit. The only real solution to this is to just get rid of them and go to single-payer, universal coverage.
