After further research from numerous sources in and outside of the military i have found what I believe to be an accurate account. He was drunk, didn’t bend over the pins on his grenades, and fumbled one of them. Rather than jumping away from it he reached for it. I was wrong and now I am admitting it. I apologize for my mistake and for anyone I offended.
